How we write these
Each case starts from the official investigation report, and every finding is credited to it. Where the report could not establish something, the case says so and leaves it open.
We do not assign blame. The lessons are written for the crew: what to check, brief or ask on your next flight in a similar spot. The sources, report first, are listed at the bottom of every case.
For study only. Always use the approved AFM, QRH and operator procedures.
